There is an embroidered, iron-on, rainbow patch in bold colors that says, "Crest Honor Roll," There is a certificate acknowledging that the child has had one cavity or less at the last checkup (There is a name handwritten on the certificate). Lastly, there are two letters: one congratulating the child, and the other congratulating the parent for their influence and direction (how sweet is that?!!) The patch is in mint condition. The two letters have some discoloration due to age; the certificate is mint (although it does have the written name which was the way it arrived from Crest.) The original envelope is included. It has been torn open. Patch measures 3.25" x 3". Nice, collectible items!
